COVID-19 Pfizer Vaccination Clinic for Muslim Community
Australian Islamic College is hosting a COVID-19 Pfizer Vaccination Clinic in collaboration with Department of Health WA for AIC students, AIC parents and our Muslim community. The vaccination clinic will commence from Tuesday 12th October 2021 at the Kewdale Campus Multipurpose Building.
Anyone aged 12 and over, including over 60s are eligible for the Pfizer vaccination
Step 1: Create VaccinateWA Account
Please register on the VaccinateWA website, if you have not already registered. You just need to complete the registration, please don’t book an appointment with any clinic on VaccinateWA website.
Step 2: Download and complete the COVID-19 Vaccination Consent Form
Please download the COVID-19 Vaccination Consent Form. You can complete the form using your laptop as a digital copy, or you can print it out and fill it. After completing the form, please email it to info@aic.wa.edu.au.
If you are an AIC parent or member of the wider Muslim community, please email with subject Covid-19 Vaccine – Parent / Community. If you cannot email, you can always bring a print out of the Consent Form with you. The form will also be available at the Australian Islamic College Vaccination Clinic, which you can fill out on the day of your appointment.
If you are an AIC student, or filling the consent form for an AIC student, please email with subject Covid-19 Vaccine – AIC Student.
Important Note
If you are an AIC student, or parent booking for your child who is an AIC student, you don’t need to complete Step 3. Your child will be automatically booked by College staff to attend an appointment during school hours, if they have completed Step 1 and Step 2.
Step 3 is only required to be completed by AIC Parents who are getting vaccinated themselves and members of the wider Muslim community.
Step 3: Book your appointment using Australian Islamic College’s Vaccination Clinic Online Booking System
If you are a parent or member of wider Muslim community, the last step is to book appointment for yourself by selecting a date and time in the Online Booking System for Australian Islamic College Vaccination Clinic. Once you book the appointment, you will get an email confirmation. Please bring that email on your phone or as a print out on the day of your appointment.
Please do not book appointments for your children studying at Australian Islamic College, as the college will be auto-scheduling their appointments between 10.30am to 3pm.
